Clinical Management of a Fused Upper Premolar with Supernumerary Tooth : A Case Report

Abstract
I. Objective
A term ¡°Fusion: in dentistry¡± is a developmental disorder of dental hard tissues. Fusion with supernumerary teeth in the permanent dentition often occurs in incisor-canine region, however, few cases of premolar fusion have been reported. The purpose of this article is to report a rare case of a fusion between a premolar and supernumerary tooth in a 13-year-old girl.

II. Case report
A 13-year-old girl was referred to Department of Conservative Dentistry at Kyung Hee University Dental Hospital with the chief complaint of pain on a pulpotomized maxillary left second premolar. CBCT image showed crown fusion of the second premolar and the supernumerary tooth. Based on the clinical and radiographic findings, the tooth was diagnosed as fusion between the maxillary second premolar and a supernumerary tooth. Root canal treatment and subsequent biscuspidization was planned. Electrocauterization of the interdental gingiva between second premolar and supernumerary tooth was done. The second premolar was prepared for with metal ceramic crown and the supernumerary teeth with gold crown. At the end of the treatment, the patient¡¯s symptom was resolved and the second premolar and supernumerary
tooth were still maintained after a follow-up examination period of 9 years.

III. Conclusion
It is important to should be aware of the anatomical anomalies in the management of fusion with supernumerary tooth, so that careful clinical and radiographic examination including microscope and CBCT is helpful to successful diagnosis and treatment. Fused teeth can be successfully managed by comprehensive treatment including endodontic and periodontal treatments.
